David O'Connell, PT, MTC, Owner
David is a certified Manual Therapist, educated in England. During the past 15 years, he has honed his skills through extensive continuing education both abroad and here in Chicago. He has lectured abroad on cervicogenic headaches and treatment of spinal disorders.

David is a former college and club rugby player in England and Ireland, who won a national championship with the University of Northumbria.

Luis A. Maldonado, PT, Owner
Luis has been practicing physical therapy for 11 years. He received his undergraduate degree in Health Education from California State University at Northridge and later attended the H.V. Amsterdam School of Physical Therapy in Amsterdam, Netherlands.

Luis has specialized in Industrial Rehabilitation for the last six years. He is the Director of the Industrial Rehab Program at Elite and leads Elite's Business Development and Marketing efforts.

An avid runner, Luis has completed two Chicago Marathons—in 2003 and 2005. He also is a mountaineering enthusiast, summiting both Mt. Rainier and Mt. Shasta. Luis lives in the South Loop and enjoys running into past patients while walking his dog Chico. "We treat everyone like neighbors because they are our neighbors."

Jeffrey D. Goode, MPT, Director of Operations
Jeff graduated from the University of Iowa with a Masters in Physical Therapy (MPT) in 1992. Since that time, he has immersed himself in Chicago’s Outpatient Physical Therapy community, specializing in General Orthopaedic and Sports Rehabilitation. He has over a decade of experience specializing in Industrial Rehabilitation services such as Functional Capacity Evaluations, Work Conditioning, and both On-Site Job Analysis and Return to Work Programs.

Additionally, Jeff has several years of administrative experience in the Chicago Outpatient Physical Therapy market. He was brought on as a team member of Elite Physical Therapy in 2005 to help manage its successful, growing business.

Amelia Hollenberg, ATC
Amelia graduated from the University of Tennessee at Martin with a Bachelor of Science in Health and Human Performance with a concentration in Athletic Training in 2008. Here she gained experience working with college level athletes participating in a variety of sports including football, volleyball, soccer—and yes, even the men and women’s rodeo teams!

Amelia comes to Elite Physical Therapy after completing an Athletic Training internship at the University of Illinois at Chicago where she worked with the Women’s Volleyball and Softball teams, as well as assisting with Men and Women’s Swimming and Diving, and Men and Women’s Gymnastics. Amelia currently resides in the South Loop, is a Certified Athletic Trainer with an Illinois license and is a member of the National Athletic Trainers Association. She is certified in CPR/AED for the Professional Rescuer with the American Heart Association.

   

David Graeff, ATC, LMT, NSCA-CPT
David graduated from Carthage College in 2005 with a degree in Athletic Training. After college, he pursued a degree in Massage Therapy from the Chicago School of Massage Therapy, in addition to becoming a Certified Personal Trainer through the NSCA. David joined Elite Physical Therapy in 2006 and is currently our Director of Athletic Training.

Alex Mena, ATC
Alex graduated from Northern Michigan University in 2004 with a BS in Athletic Training. He is NATABOC certified and speaks both English and Spanish.

Alex enjoys working with people of all ages. He specializes in working with sports and the return of athletes to their sports after injury. He incorporates this specialty into Industrial Rehabilitation by helping his patients return to work after a work-related injury. He also uses work-specific and functional exercises to help prevent further injury once they return to work. He takes his job seriously, but also believes in making therapy fun. He is demanding of his patients, and tries to be as inspirational as he can to push them to reach their goals.

During his off time Alex enjoys riding his mountain bike on the beachfront, kayaking, working out and everything that comes with summertime (including cook outs almost daily). During the long winters in Chicago he enjoys snow boarding and snowshoeing in the upper peninsula of Michigan with old friends from college. With all his activities, he still somehow finds a way to cook out in the winter a few times every year!

Eric De Porter, ATC
Eric is a recent graduate of Eastern Illinois University where he received his degree in Athletic Training. While at EIU, Eric covered Division 1 football, cross country, track & field, baseball and rugby. He also worked with the Saint Louis Rams as a pre-season intern in 2008, and traveled to Haiti on a medical mission in 2009.

Eric enjoys a variety recreational activities such as slow pitch softball, golf, wake boarding/water skiing, snow boarding and mountain biking.

Amy Garman, ATC, MED, PES, Athletic Trainer
Amy is a Certified Athletic Trainer who spent the past five years as an Athletic Trainer for Arizona State University, gathering valuable experience working with the NCAA Division I and nationally-ranked gymnastics, football, tennis and volleyball teams. She also recently completed her Performance Enhancement Specialist training through the National Academy of Sports Medicine.

Amy is currently in a consulting position with Elite Physical Therapy.

Rebecca Gardner-Reilly, MPT
Rebecca graduated with honors from the University of Missouri-Columbia with a Master’s Degree in Physical Therapy in 2004. After graduating, Rebecca practiced physical therapy in Seaside (OR), Seattle, Houston, Dallas and now Chicago. In those various locales, she worked in a variety of settings including acute therapy and inpatient rehabilitation, as well as land and aquatic-based outpatient therapy.

Though she has thoroughly enjoyed her travels and past work experiences, Rebecca is looking forward to planting some roots at Elite and narrowing her focus to outpatient therapy. Manual therapy has been the primary focus of the continuing education classes Rebecca has taken. She plans to continue this field of study, as well as pursue continuing education coursework in Women’s Health.

A new resident of the South Loop, Rebecca loves not only the short commute, but also the great views and restaurants the neighborhood has to offer. Recreationally, she likes to camp, canoe and kayak, snow ski and is learning to sail.

Matthew N. Butler, DPT, CSCS
Dr. Butler received his Doctor of Physical Therapy (DPT) Degree from the University of Miami’s Miller School of Medicine where he graduated with honors in 2005 and received the Steven J. Rose Memorial Endowed Scholarship for Excellence in Research. He received his certification as a Strength and Conditioning Specialist from the National Strength and Conditioning Association and his certification as a Pilates Rehabilitation Practitioner from Polestar Pilates.

Dr. Butler is a Manual Therapist specialized in treating sports and orthopaedic injuries. He completed his manual therapy training through Maitland-Australian Physiotherapy and the Orthopaedic Physical Therapy Institute of Spinal Manipulation and Sports Medicine. He has been a contributing author for the fifth edition of Neurology and has co-published several abstracts on the effectiveness of Pilates-based exercise on low back pain in the Journal of Orthopaedic and Sports Physical Therapy.

Dr. Butler is currently on leave, pursuing a fellowship at UIC. He continues to work with Elite Physical Therapy on a research project in partnership with the Columbia School of Dance.
